Synopsis:
Addison's patients must choose which one of their sickly daughters to save with their newborn's core blood; Sam discovers that Dink intends to marry Maya.

Addison and Cooper counsel a couple who must make an unthinkable choice, to decide which one of their ailing daughters to save with their newborn’s core blood. Sam is dealt another blow when he finds out Dink’s intent to marry Maya, and the rest of the gang runs an intervention on a still distant Naomi.
We know it’s always hard for a dad to give his daughter away to another man. The fact that the guy’s name is Dink can’t make things any easier. Sam decides to drown his sorrows at a bar with the boys. But when Pete and Cooper ditch him for various reasons, he ends up getting drunk solo. He pops by Addison’s place for a nightcap that ends with a passionate kiss. Addison pulls away because Sam is drunk. Sam says, “I’m not that drunk.”
The next day Addison and Sam try to analyze the kiss in her office. That just leads to more kissing. Well, almost. Sheldon knocks on the door for some smoochis-interruptus. The good news is that Sheldon has managed to reel in Naomi for a discussion about the Maya situation. But Naomi is still a mess. Sam says that when he meets with Corinne again, he’s going to have to start making some decisions for the both of them.
Addison tries to talk to Naomi and subtly broaches the subject of her and Sam as a couple. This completely cracks up Naomi. She laughingly says, “First you sleep with your husband’s best friend, then you sleep with your best friend’s husband. You would be the worst person ever!” All Addison can do is laugh it off, too. Well, on the outside, at least.
Naomi does end up meeting with Sam, Corinne and the kids. But she arrives a little late. Some of those tough decisions Sam mentioned earlier have already been made. He signed the consent form allowing Dink and Maya to marry. By the way, Dink’s real name is Fillmore. We assume that was the name he used on the paperwork. Naomi is still a wreck, but a heart-to-heart chat with Violet seems to help.
Speaking of Violet, she’s still adjusting to life with her new roomie, Charlotte. Cooper is adjusting, too. It’s gotta be hard to have your ex-girlfriend sharing a space with your best friend. Charlotte tells Cooper that her new living arrangement has nothing to do with their relationship. He doubts it at first, but eventually promises not to make the situation all about him, which is nice.
Addison and Cooper are dealing with a couple who had a baby to use the infant’s blood to save their twin daughters who have leukemia. The problem is the cell volume is low, so there’s only enough blood to save one of the eight-year-old girls. To complicate matters, neither parent wants to see their new baby boy. It’s almost as if they blame him for this terrible dilemma.
Faced with an agonizing decision, the mother can’t bear to pick one daughter over the other. The father, however, can’t watch them both die. One of the girls, Nicki, develops an infection. It happens to be the girl that the father opted not to save. Coop tells him that Nicki is in worse shape than her sister, Kelly. Without the transfusion, she’ll die. But the father knows that Kelly will then eventually be the one who dies.
The impossible decision is made for the parents when we find out that Nicki is too sick to receive the treatment. It’s up to Coop to break the news to the girls. Kelly doesn’t want the blood if Nicki can’t have it, too. But Nicki knows that her sister has a chance to live. She wants her to take it. It’s heart-wrenching television. But the tears really start flowing once Nicki’s mom brings in the baby so she can spend some time with her little brother.
At the end of the day, Sam visits Addison at her place once again. He wants to take their relationship to the next level. Make that any level. Sam takes Addison in his arms. As much as Addison may want Sam, the one thing she doesn’t want is to hurt Naomi. As she pushes Sam away, the only thing Addison can say is, “I’m sorry.”
